LIABILITY WAIVER & RELEASE OF CLAIMS

PLEASE READ THIS DOCUMENT CAREFULLY BEFORE PROCEEDING. THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR RIGHTS.

Activity: Participation in classes, programs, events, and activities offered by The Adventure Library, including but not limited to: sensory play, crafts, movement activities, and play (hereinafter collectively referred to as "the Activities").

Participant/Guardian Information: This Release applies to myself, my child(ren), minor(s) for whom I am a legal guardian (hereinafter "Participant").

Acknowledgment of Risks: I understand that participation in the Activities offered by The Adventure Library involves inherent risks, hazards, and dangers that can result in injury, illness, permanent disability, or death. These risks may include, but are not limited to, physical exertion, interaction with equipment, tools, or materials, contact with other participants, varying environmental conditions, or unforeseen events. I understand that The Adventure Library cannot guarantee a risk-free environment.

Voluntary Participation & Assumption of Risk: I acknowledge that my (or the Participant's) participation in the Activities is entirely voluntary. I freely and voluntarily assume all risks, both known and unknown, foreseen and unforeseen, inherent and otherwise, associated with participation in the Activities, even if arising from the negligence of The Adventure Library or others.

Release and Waiver of Liability: I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue The Adventure Library, its owners, directors, officers, employees, volunteers, agents, and insurers (hereinafter "Released Parties") from any and all claims, demands, losses, or damages, including those caused by the negligence of the Released Parties, arising out of or in connection with my (or the Participant's) participation in the Activities.

Indemnification: I agree to indemnify and hold harmless the Released Parties from any and all liabilities, claims, demands, actions, or causes of action, including attorneys' fees and costs, asserted by any third party as a result of my (or the Participant's) actions or involvement in the Activities.

Understanding and Agreement: I certify that I have read this entire document, understand its content, and acknowledge that by proceeding with registration (ticking the checkbox), I am agreeing to its terms. I am at least eighteen (18) years of age or am the legal parent/guardian of the Participant(s) named in this registration and have the authority to sign this Release on their behalf.
